The importance of pharmacist to pharmacist collaboration across health care settings has never been more important as patients transition throughout the health care system. This course will highlight an example of collaboration between community pharmacists and clinic based pharmacists. Lessons learned and opportunities to enhance patient care will be discussed.
 
Learning Objectives
Upon successful completion of this knowledge-based CPE activity, participants should be able to:
 
1. Review the challenges to achieving optimal patient outcomes across practice settings.
2. Describe opportunities to improve patient outcomes by enhancing pharmacist to pharmacist collaboration.
3. Describe the community pharmacist to clinic based pharmacist CPESN collaboration.
4. Identify examples of improving patient care through pharmacist to pharmacist collaboration.
